Transition to Adult Care and Support

 

The Transitions Team support disabled young people under the age of 18 to prepare for the transition to adult services. They also provide support to adults up to the age of 25 who have an on-going Education, Health and Care Plan (EHCP). Once an adult has completed their education and are living in settled accommodation, they will be transferred out of the Transitions Team to the relevant social work team.

When working with young adults (18-25 years) the Transitions Team carry out a range of functions under the Care Act 2014 and the Mental Capacity Act 2005 (from 16 years) including assessment of needs and/or mental capacity, Care and Support Planning, review and best interest decision making.

The Team staff a Duty Service for those individuals who do not have an allocated worker. The Duty Service can be contacted via LDDuty@Swindon.gov.uk or 01793 464820.

The Team consists of a Service Manager, Team Manager and Assistant Team Manager working alongside Social Workers, Assistant Care Managers and Link Workers.

Note: Practitioners carrying out adult social care functions, such as Care and Support Planning should use the procedures for the team that the young person will be transferring to.
