Review and Transformation Team
The Reassessments team's primary focus is to provide an efficient, professional and timely service and alleviate stress on service users and carers where ever possible, ensuring there are no safeguarding concerns.
The role of the Review and Transformation Team then, is to carry out reassessments on service users that are already in receipt of care from an initial assessment, in a variety of situations in order to determine ongoing eligibility under the Care Act 2014 and meet identified needs in the most appropriate way. Reassessments are necessary to review someone's existing needs, as the vulnerable nature of the people served means that someone's needs can change very quickly.
The team, based on completed reassessments, must also ensure any changes in assessed eligible needs are reflected on support plans which can then lead to a change of services then, depending on the service user's circumstance, this can lead an increase or a decrease.
