ASC Teams
ASC North Team
The Adult Social Care Team North are based within Southport Town Hall, which is on Lord Street. Although we are on the top floor, we have rooms downstairs to meet individuals where needed.
The team consists of a Team Manager, Lead Practitioners, Admin, Social Workers and Community Care Practitioners.
We actively encourage student placements within the team, as this gives additional opportunities to reflect and develop our own practices.
The aim of the team is to complete assessments of need in line with our statutory duties for vulnerable adults aged 18 upwards, 16 onwards for young people coming through the transitions process. The team aims to provide a timely quality asset-based assessment to reduce risks and to support individuals and carers to reach their full potential providing where assessed as needing a support package to meet individual eligible needs.

Note: The procedures on this page apply when working with adults (post 18). Anyone supporting a 16-18 year old through transition should refer to Transition to Adult Care and Support.
ASC South Team
The team provides social care services for adults over the age of 18. The core purpose of adult care and support is to help people to achieve the outcomes that matter to them in their life.
The service delivers high-quality assessments and arranges care and support that meets the eligible needs of the most vulnerable people in Sefton, through the commissioning of individual support plans and packages of care that leads to improved outcomes for individuals, their families and Carers.
