Learning Disability and Autism Team
The Learning Disability and Autism Social Work Team are based on 1st Floor at Magdalen House, Bootle.
The team consists of a Team Manager, Lead Practitioners, Social Workers and Community Care Practitioners.
We also actively encourage student placements within the team, as this gives additional opportunities to reflect and develop our own practices.
The aim of the team is to complete care act assessments, support plans, and reviews in line with our statutory duties for adults with learning disabilities and/or autism aged 18 upwards and their Carers. Our team also contains within it, a 14-25 transition team which supports young people and their families as they move into adulthood.
Our team is being co-produced with people with lived experience from People First.
The team aims to provide timely quality strength-based person-centred assessments to support individuals and their carers to reach their full potential and be able to participate in their local community by providing, where assessed as needing, a support package to meet individual eligible needs.
We work very closely with the LD Health Team in Merseycare.
