Southport Hospital and ICRAS
The Southport Hospital Team are a team of Social Workers based at Southport District General Hospital. The team currently consists of eleven practitioners (two of which cover the ICRAS Integrated Community and Reablement Assessment Service), one Team Manager and one Lead Practitioner. The team complete assessments under the Care Act and work alongside Health colleagues in the Trust to facilitate safely and timely discharges of patients from Hospital.
Whilst relatively small, the hospital is extremely busy due to the size and acuity of the population it serves. There are twelve wards. All our practitioners are ward allocated, therefore the expectations are that practitioners attend the daily ward meetings. Of a weekday the team provide cover for the core local authority hours and operates a Duty Social Worker system. The Local Authority also has practitioners working of a weekend to facilitate discharges wherever possible.
