Aintree Hospital
The Social Work team at Aintree Hospital facilitate safe and timely discharges from the Trust in accordance with statutory duties under the Care Act 2014 and Mental Capacity Act 2005. Social Workers complete Social Care Assessments, Mental Capacity Assessments, attend and chair, or contribute to Best Interests Meetings. The team are also allocated to wards to support discharges through providing information and advice.
Where possible, Social Workers facilitate discharges to a persons' own home with a Reablement Service or package of care, or a Social Worker may need to arrange a short-term placement in James Dixon Court in a transitional or enabling bed.
The team also work alongside the Merseycare Assessment Team to arrange discharges to a 28-day bed which is a Nursing Home placement. Furthermore, Social Workers also arrange 24hr care placements in residential and nursing homes.
