4.3.4 Transition for Young People with Learning Difficulties / Disabilities in Rochdale

Please note that these processes are currently being reviewed and developed to ensure they reflect the forthcoming changes to Special Educational Needs / Disability legislation (Introduction of a Local Offer and Education, Health and Care Plans up to the age of 25 for those young people who remain in education and/or training).

1. What is Transition?

  • This is Transition Team in Rochdale;
  • The Learning Disability Transition Team is a joint team made up of a social worker and a support planner from RMBC Adult Service and a nurse from Pennine Care Foundation Trust. They work closely with services for young people with significant learning difficulties / disabilities and their families during the transition phase.


2. What Does the Transition Team Do?

  • The Transition Team provide advice and information to young people with learning disabilities (aged 13+) and their families during their transition from Children’s to Adult Services;
  • Support transfer of Health Services during the transition period and offer access to Health Plans such as Health Action Plan’s, Traffic Light Assessment, Continuing Health Care Assessment, Health Assessment, transfer of equipment into adult services, ensure transferee of consultants, referral to allied professionals for health needs. Arrange training for adult provision and education placements. Provide families and service users with information with regards individual health needs. Provide health promotion sessions and person centred planning;
  • Multi-agency work with all health, education, service providers and social service agencies and professionals involved with the young person to ensure that young people and their families receive a smooth and cohesive transition between services;
  • Inform future service provision and enable the commissioning local services to meet the specialist needs of young people in transition;
  • Joint work with Children’s services early intervention and prevention work placing young people in adult placements early to develop long term sustainable placements in the borough;
  • Working with young people who meet the criteria for involvement who are placed Out of Area; 
  • Assess for eligibility to determine most appropriate pathway into most appropriate  adult services;
  • Assess for personal budgets and arrange any potential Adult Care Services that may be needed and eligibility for Continuing Health Care Funding;
  • Support plan with families to ensure that needs are met;
  • Assess carer need;
  • Provide information to families on wide range of issues – benefits, FACS criteria, legislation, community opportunities, transport, housing, employment, adult services, health services;
  • Facilitate annual transition ‘Moving on day Event’ to provide parents and service users with access to wide range of services;
  • Contribute to service development in the local area.


3. How Does the Referral Process Work?

  • A referral can be made to the transition team from age 14 or from the year 9 review, information is then held on a data base with the level of involvement  determined on an individual basis dependent on needs.
