How to Access Mental Health Social Work

1. Making a Referral

CMHT

Referrals should be made via the Common Point of Entry by the person's GP.

There are other sources of referral, for example, when a person is assessed under the Mental Health Act and detained in hospital or as a result of a Safeguarding Concern.

Bracknell Forest Council cannot make a direct referral into the team.

CMHTOA

Referrals should be made via:

• The Common Point of Entry by the person's GP; or
• The Local Access Point (LAC).

There are other sources of referral, for example, when a person is assessed under the Mental Health Act and detained in hospital or as a result of a Safeguarding Concern.

Bracknell Forest Council cannot make a direct referral into the team.
