How to Access the ICS
1. Who can the ICS Support?
The Intermediate Care Service can support any adult from the age of 18.
This includes adult's experiencing physical challenges, cognitive difficulties or mental health issues.
2. Making a Referral
Hospital Discharge
Referrals can be made by ward staff following BHFT processes.
Where involved, the Hospital Discharge Social Work Team can also make the referral. To do this, the referral form on LAS should be completed.
Community Teams
Any Bracknell Forest Council adult social care team can make a referral using the referral form on LAS. This includes:
- The ACT Team (duty or long term);
- The CTPLD;
- The Autism Team; and
- The CMHT or CMHTOA.
Self-Referrals
It is not possible to self-refer into the Intermediate Care Service. Anyone wishing to explore reablement should contact the customer service team who will be able to process any referral should this be deemed to be the most appropriate and proportionate course of action.
