Community Team for People with a Learning Disability
Introduction to the CTPLD
The CTPLD are a joint health and social care team that supports adults with learning disabilities and their carers. The team is made up of the following professionals:
- Social Care Team including social workers and care co-coordinators;
- Challenging behaviour specialist;
- Learning disability nurses;
- Psychologists;
- Occupational Therapist;
- Dietician;
- Physiotherapist;
- Speech and language therapist;
- Support workers.
Remit of these procedures
The procedures in this section are relevant to Bracknell Forest Council practitioners on the CTPLD. Employees of Bracknell & Ascot ICB should refer to available procedures provided by the ICB.
The procedures should be used alongside any ICB procedures that BFC employees are required to follow.

The procedures in this section are specific to adults who have (or may have) needs for care and support. Practitioners working with carers should refer to the dedicated procedures: Carers.
