Adult Community Team

Introduction to the Team

The Adult Community Team (ACT) works predominantly with adults over 18 who may have a long term physical health condition or disability or who have eligible care and support needs through older age and associated health and physical impairment.

ACT encompasses a Duty Function with senior staff managing this initial intake phase.

The Duty function is also responsible for general daily enquiries or referrals for social work, admission avoidance or Occupational Therapy. There are a number of dedicated staff within the Duty function who will take these referrals and carry out the first element of the 3 Conversations process, which is a model employed by Bracknell Forest Council.

Further to the Duty function is the Community Team, Hospital Team and Intermediate Care Team.

These Teams are responsible for other aspects of Community social care and this may include Continuing Health Care (CHC) assessments, reablement, Safeguarding, reviews of placements and ongoing case management.

The procedures in this section are relevant to local authority social work and OT practitioners on the team working with adults who have (or may have) needs for Care and Support.
