AMENDMENTS
This Chapter was amended significantly in August 2011 adding links to relevant documentation for Regulatory Authorities and to take account of a NEW chapter added to the manual: Event Notifications Table; these amendments overwrite those applied previously in April 2009, relating to the senior managers who must be notified.

1. Definition of Notifiable Events
Any Incident must be reported to the Home's Manager, Line Manager, Social Workers, as set out in individual Chapters e.g. Physical Interventions Procedure.
However, some Incidents are also defined as 'Notifiable Events'.
Notifiable Events are listed in the table below.
2. Notifications and Timescales
All notifications must be made without delay, initially be made verbally, and confirmed in writing, see Section 3, Recording of Notifiable Events.

3. Recording of Notifiable Events
Notifiable Events must be recorded on the 'Notification of Significant Incidents Form' In addition any notification forms as required by Ofsted or CSSIW must be completed.
